In order to obtain the most up to date records for you, including any new medical conditions I am requesting that all clients, existing and new to complete this new form within 2 days of your appointment. This incorporates the COVID19 risk assessment, which is a mandatory requirement. If you have difficulty completing the form please contact us.
In order to maintain the strictest hygiene within the cabin I have created a COVID19 policy. It is important for all visitors to read the policy, so you understand what is expected and what to expect when you arrive.
